Family Medicine Associates of South Attleboro was founded at 230 Washington Street in South Attleboro by Dr. Joshua Gutman. Dr. Gutman, board certified in Family Medicine and Internal Medicine, first opened the office doors on July 1, 1978.
Since that time, the number of physicians and office staff has grown considerably. Dr. Gutman officially retired from the office (but not from the practice of medicine) in 2006. Under the direction of Dr. Daniel Brown a second office was opened at Tower Square in North Attleboro in 2008. On October 1, 2013, the original location was closed and a brand new office was opened just down the road at 562 Washington Street in South Attleboro.
Here are a few important office policies we would like to share with you.
Payment for your visit is expected at the time of your appointment. Some visits, such as physical exams, may not require a payment. However, if you have a co-pay we will collect it at the time of the visit. Paying your co-pay at the time of your visit is an obligation you have with your insurance company. It also helps us to continue to provide the excellent medical care you expect from us. We accept cash, check or credit card.
If you are unable to pay your bill at the time of the visit, our financial department will work out an alternative payment arrangement for you.
Often there is some type of form to be filled out in connection with your visit. Whether a surgical clearance form, a form for work, an immunization form for school or a note stating that you were seen in our office, you can help us by bringing the form with you or telling us what you need during the visit. Forms brought to us outside of an office visit may take some time to complete and be subject to a fee.
Our office is affiliated with Steward Health Care. If you need referrals you usually will need to stay within this Steward Network. If you are not sure you should ask your provider. We ask for a 5 day notice prior to your appointment with a specialist in order to generate a referral, but understand that urgent visits are sometimes needed. Contact us right away either by portal or on our referral line.
We have an automated call system that contacts you by phone or email to remind you prior to your appointment. We understand that emergencies and other situations may arise that necessitate cancelling and/or rescheduling your appointment. If you need to change an appointment please contact our office at least 24 hours in advance. Failure to do so may result in a "no-show" fee of $50.00.
